Details of Experience:
Mr. Wiseman is a 62-year-old male who was admitted to The Hamlet Rehabilitation and Healthcare Center at Nesconset on July 3rd after his stay at Syosset Hospital for a bilateral total knee replacement. Within the first 12 hours of admission to The Hamlet Rehab and Healthcare Center at Nesconset, Mr. Wiseman was evaluated by Occupational and Physical Therapy and seen by clinicians, administrative staff and other valued members of the team.

Mr. Wiseman was notified that he would need inpatient rehabilitation to support his range of motion, ability to ambulate and perform activities of daily living. During his Physical Therapy sessions, Mr. Wiseman worked on increasing functional activity tolerance, enhance fall recovery abilities, improve bed mobility and transfers and improve dynamic balance.

Upon admission, he was unable to ambulate, ascend and descend stairs or perform bed mobility tasks. He needed maximum assistance for weight shift over the center of gravity. After the first week, Mr. Wiseman was able to perform functional transfers with contact guard assistance and was able to safely ambulate on level surfaces 100 feet using the rollator walker. By week three, he exhibited normalized gait patterns while safely ambulating 200 feet with independence without an assistive device and maintaining good balance. At discharge, he was able to ascend/descend 12 stairs with independence.

With the teamwork and cooperation of Team Hamlet, on July 22, 2019, Mr. Wiseman was discharged to his home. His ambulation increased from 0 to 200 feet and he was able to transfer independently and safely ascend/descend 12 stairs with independence.

Mr. Wiseman was able to achieve his goals due to his fantastic therapy given by John and Kurt and the rest of the team. He thought the staff was outstanding and that the new rooms were beautiful. He rated his overall stay a 10. We wish Mr. Wiseman the best of luck ahead now that he is home with his wife and children.
